There are two types of dysmenorrhea, primary and secondary. Understanding these different types can help you better understand where your pain may be coming from, what you can do about it, and give you more information so that you can <\/span><a href=\"https:\/\/www.intimina.com\/blog\/self-advocate-for-medical-care\/\"><span style=\"font-weight: 400;\">advocate for yourself<\/span><\/a><span style=\"font-weight: 400;\"> in medical settings.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">What is the difference between primary and secondary dysmenorrhea? We\u2019re here to explain.\u00a0<\/span><\/p>\n<h2>Symptoms of Dysmenorrhea<\/h2>\n<p><span style=\"font-weight: 400;\">People typically associate cramps with period pain. For those who experience dysmenorrhea, it often doesn\u2019t stop with cramps. Painful periods can also come with a whole slew of other symptoms like:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pelvic pain<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Lower back pain<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cramping or pain in the lower abdomen<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nausea\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Vomiting<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Headaches<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Dizziness<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pain that radiates down the legs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fatigue<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fainting<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Experiencing intense pain and uncomfortable symptoms for multiple days a month can also have a severe impact on someone\u2019s mental health. Many people with dysmenorrhea may also experience symptoms of <\/span><a href=\"https:\/\/www.intimina.com\/blog\/pms-vs-pmdd\/\"><span style=\"font-weight: 400;\">PMS or PMDD<\/span><\/a><span style=\"font-weight: 400;\">, along with anxiety or hopelessness around being in so much pain every month.\u00a0<\/span><\/p>\n<h2>Primary Dysmenorrhea<\/h2>\n<p><span style=\"font-weight: 400;\">Cramps that come before or during your period are what\u2019s called primary dysmenorrhea.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">They can happen before or during your period and are caused by the hormone-like substance <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/articles\/24411-prostaglandins\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">prostaglandins<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Prostaglandins help with a wide range of bodily functions like blood flow and labor. They also cause the muscles and blood vessels in the uterus to contract during menstruation. This helps to release endometrium (uterine lining) from your uterus, aka your period.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Prostaglandin levels tend to be higher at the beginning of your period and taper off towards the end. Hence many people experience more pain at the start of their period, which tends to subside after a few days.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Primary dysmenorrhea typically starts within six to twelve months of the start of menarche (an adolescent\u2019s first period), and can often get better with age.\u00a0<\/span><\/p>\n<h2>Secondary Dysmenorrhea<\/h2>\n<p><span style=\"font-weight: 400;\">Secondary dysmenorrhea on the other hand is period pain that is caused by a reproductive health disorder, pelvic pathology, or other medical condition. This type of period pain may start a few days before the beginning of a period, can get worse throughout each cycle, and may even continue at the end of each period.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This type of period pain can be so severe that it interferes with daily activities for several days per month. Endometriosis is the <\/span><a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/committee-opinion\/articles\/2018\/12\/dysmenorrhea-and-endometriosis-in-the-adolescent#:~:text=Primary%20dysmenorrhea%20characteristically%20begins%20when,of%20secondary%20dysmenorrhea%20is%20endometriosis.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">most common cause<\/span><\/a><span style=\"font-weight: 400;\"> of secondary dysmenorrhea.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Secondary dysmenorrhea can also be caused by:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fibroids<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Congenital defects in the reproductive organs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Tumor, infection, or polyps in the pelvic area or reproductive organs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.intimina.com\/blog\/what-is-adenomyosis\/\"><span style=\"font-weight: 400;\">Adenomyosis<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.intimina.com\/blog\/pcos-polycystic-ovary-syndrome\/\"><span style=\"font-weight: 400;\">Polycystic Ovary Syndrome<\/span><\/a><span style=\"font-weight: 400;\"> (PCOS)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Miscarriage, pregnancy loss, or an ectopic pregnancy<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">While less common, adolescents can have secondary dysmenorrhea. Endometriosis in adolescents can be difficult to diagnose because the lesions may be clear or red and harder to identify than those in an adult.\u00a0<\/span><\/p>\n<h2>Treating Dysmenorrhea<\/h2>\n<p><span style=\"font-weight: 400;\">Treatment for dysmenorrhea depends on the root cause of the pain, and if it\u2019s primary or secondary. Your OB-GYN will most likely perform a pelvic exam, followed by blood tests and possibly an ultrasound.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For cases that need further diagnosing, a laparoscopy may be performed. This is a minimally invasive procedure that involves making a small incision near the belly button to allow a laparoscope, or thin lighted camera, into the abdomen to explore what\u2019s going on inside.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Primary dysmenorrhea can often be managed through over-the-counter medications like ibuprofen, naproxen, and acetaminophen, as well as using a heating pad when you have pain. Some people may find relief through dietary and lifestyle changes, and by taking certain supplements like <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28392498\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">magnesium<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Secondary dysmenorrhea is more complex because it can be caused by a wide range of disorders. This is especially essential in reproductive health care, an area where women\u2019s bodily autonomy is so often stifled.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you\u2019re experiencing period pain (or you just want better health care), here are a few tips to better advocate for yourself:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Track your symptoms, energy levels, and anything else that may be connected to what you\u2019re experiencing.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Do your research ahead of time. It shouldn\u2019t all be on you, but being prepared helps you know what your options are.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Come with a list of questions. It\u2019s easy to forget them when you\u2019re being rushed through an appointment, so coming with a list will help you cover all your bases.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Don\u2019t be afraid to speak up.
